From the 8th day onwards, the doctors may provide sickness certificates, if requested, following consultation. If you have been off work for some time the DSS may ask you to obtain form MED3 from your doctor Please make a routine appointment for this. Following consultation, the form will usually be available for collection within a few days. There is no charge for this.
