What is One Health and Care?
‘One Health and Care’ is a new integrated care record for individuals registered at Staffordshire and Stoke-on-Trent GP practices that brings data together from the different organisations involved in health and social care.
Completely confidential and secure, it’s designed to help doctors, nurses and other registered health and social care professionals directly involved in a patient or client’s care to make better, safer decisions.
What information will be available on One Health and Care?
The information that the professionals directly involved in a patient or client’s care will be able to see includes:
Their information will be held securely and only shared with staff members in the partnership organisations who are directly involved in their care and support.
What information will NOT be available on One Health and Care?
Sensitive information such as attendance at sexual health clinics, fertility treatment records, and records relating to gender reassignment will not be included.
What are the benefits of One Health and Care?
One Health and Care will enable professionals to provide better care by ensuring they have access to the most up-to-date information about their patients or clients.
As a result, patients and clients won’t have to keep repeating their health and social care history to health and social care professionals. The results of common tests (for example blood tests) will be available to everyone involved in their care, reducing the need to repeat them.
This will help improve decision making to give patients and clients safer, more consistent care whether they are in hospital, at a GP Practice or any other place where they receive care.
Which organisations will provide data for One Health and Care?
If a patient or client is happy for their information to be shared as part of One Health and Care, do they need to do anything?
No. If a patient or client is happy for their information to be shared, they do not need to do anything as we will do it all for them.
Can patients or clients opt out of One Health and Care?
Yes, they can. To opt out, patients and clients need to contact their GP Practice and ask to opt out of the local digital shared care record One Health and Care. Their GP Practice will then handle this process on their behalf.
If they choose to opt out, their information will not be available via One Health and Care however it will continue to be shared by care organisations by phone, email and on paper where required as part of their direct care.
Please advise patients or clients to consider carefully before opting out as it could mean that vital information about them is not immediately available when they require health or social care support.
If a patient or client has already opted out of the national Summary Care Record, do they need to opt out of One Health and Care too?
No. If they previously opted out of the national Summary Care Record then they will be automatically opted out of One Health and Care and do not need to contact their GP Practice.
If a patient or client opts out of One Health and Care, can they opt back in?
Yes, they can. If a patient or client opts out of One Health and Care (or were automatically excluded as they had previously opted out of the national Summary Care Record), they can change their mind and opt back in at any time, by contacting their GP Practice and asking that their data is included.
