We are pleased to announce that the North Staffordshire Combined Healthcare NHS Trust’s online All-Age Wellbeing Portal self-referral system for adults and people with learning disabilities is now live.
This is a new capability within the Portal, enabling patients with the opportunity to refer themselves using the new online referral form.
It also allows carers and professionals to refer on behalf of the patient, where consent has been provided.
This is fantastic news!
