Chronic Disease Management

Chronic Diseases can include the following:

  • Heart Disease
  • Heart Failure
  • Diabetes
  • Atrial Fibrillation
  • Chronic Kidney Disease
  • Stroke
  • Rheumatoid Arthritis
  • Epilepsy

Each patient with a chronic disease is reviewed yearly. If patients have more than one chronic disease, they would be checked altogether.

This would tell the clinician the medication you are taking for that condition is working and can therefore perform a medication review.

Some conditions may need to have a blood test first and then a review with one of our health care professionals. Some conditions could be reviewed more than once a year dependant on the condition, blood results or examination.

Some annual reviews would be bookable with a GP.

Our team would book you in with the most appropriate clinician for the review.
